Management of Hypothyroidism in Children through Ayurveda Intervention - A Case Report

Thyroid dysfunction is an unseen rival in a child's body that hinder their growth and development. Food and lifestyle changes along with familial relationships are found to be the most important cause of thyroid dysfunction in children. A 15-year-old female child with complaints of increasing weight, hair fall and constipation, heaviness on the face and frequent muscle cramps in both lower limbs for the last 4 months. She was diagnosed and treated with Hypothyroidism. According to Ayurved classics, it can be correlated with Galaganda (goiter). The principles of Kapha Vridhhi (increased Kapha), Datwagni Mandhya (depressed metabolic factor located in bodily tissues) and Rasa-Meda Dushti Chikitsa (treatment for primary product of food digestion and fat tissue) were used, i.e., Ajmodadi Yoga, Cap. Thyrin, Gandamalakandan Rasa and Surya namaskar. The Patient showed remarkable recovery in the complaints, i.e., weight reduction, relief in constipation, hair fall and muscle cramps. Through Ayurvedic interventions, clinical symptoms were reduced and laboratory parameters changed towards normalcy. This further confirms the relationship between disease and adopted treatment principles.

Partial Fistulectomy and Ksharsutra in Complex Anterior Horseshoe Fistula-in-Ano - A Case Report

Bhagandar (Fistula-in-Ano) is explained as one among Ashta Mahagada in Sushruta Samhita. This disease is recurrent in nature which makes it more difficult for treatment. Ksharsutra has been already proved in the management of fistula-in-Ano though Acharya Sushruta explained in the context of Nadi-Vrana (sinus). A 21year old female patient came to Shalya Tantra OPD with complaints of multiple boils at perianal region associated with pain and blood mixed pus discharge since, two months. On PRE multiple external openings present anteriorly between 2 to 10 0’clock, internal opening palpated at 12 0’clock. TRUS done, suggestive of Horseshoe fistula-in-Ano. Patient was treated with partial-fistulectomy with Ksharsutra application under Sadal block. Ksharsutra was changed by weekly interval by railroad technic. Whitin 8weeks wound was healed completely and without any recurrence.

Exploring Modified Siravedha for Effective Treatment of Vatakantaka (Plantar Fasciitis)- A Single Case Report

Plantar fasciitis is one of the common causes of heel pain and it affects one in ten persons throughout their lifetime. Due to over strain to the muscles and ligaments of the plantar region, over-stretching of the plantar fascia is leading cause of the manifestation of plantar fasciitis. Looking at aetiopathogenesis, signs & symptoms, Vatakantaka mentioned under Naanatmaja Vatavyadhi can be correlated with plantar fasciitis and treatment modalities prescribed for it can be adopted for its management. Siravedha (~ venepuncture) is a unique modality given by Acharya Sushruta to let out noxious blood from the painful site and gradually cure the disease. A 39-year-old female patient visited Asthi-Sandhi-Marma Roga OPD with complaints of pain in the left heel region. Based on examination, she was diagnosed a case of plantar fasciitis and 4 sittings of Siravedha were done once weekly for 4 weeks after local Snehana (~Therapeutic oleation) and Swedana (~ sudation therapy) at foot region. Effective relief was achieved as pain and tenderness decreased significantly after each session. The Visual Analog Scale score dropped from 8 to 0, while tenderness decreased from Grade 4 to 0. Additionally, there were no recurrences during the 6-month follow-up period.

Katupila (Securinega leucopyrus) and adjuvant Ayurveda Medicines in Karnini Yoni Vyapad (Cervical ectropion with cervicitis)- A Case Study

Although cervical ectropion resolves itself over time, it creates a vulnerable ground for seeding various sexually transmitted infections and it is diagnosed on routine pelvic examination or pap screening. According to Ayurveda, signs and symptoms of the condition can be correlated with the Karnini Yoni Vyapad. Intension was to determine the efficacy of Securinega leucopyrus (Katupila) in managing cervical ectropion and cervicitis conditions. Yoni Purana and Yoni Pichu with Katupila Swarasa were administered, for a patient with the above condition following Pancavalkala Avagaha continuously for two weeks. After one week of treatment, the appearance of the discharge was significantly changed. The yellowish discharge became transparent while oedematous appearance and lower abdominal pain disappeared; wrinkled appearance and contact bleeding were not present during the examination after treatment. After the second week of treatment, a bright-red region that extended beyond the external cervical OS had further reduced, contact-bleeding and vaginal discharge had also reduced significantly Therefore, it can be concluded that the Katupila Swarasa can be used as Yoni Purana and the Yoni Pichu, to treat cervical erosion and cervicitis successfully. It is an affordable treatment which has no adverse effects.

Ayurvedic Management of the HPV Warts through Rasa Aushadhis - A Case Report

Warts are widespread in worldwide. The estimated current rate of wart among the general population is approximately 10%. HPV (human papilloma virus) is one of the common causes of warts. Most people will experience with this infection at some time in their life. An increased frequency also seen among immunosuppressed patient and meat handles and it is common in both male and female. According to Ayurveda, these warts can be compared with Charmakeela. The pathogenesis of this disease is due to vitiation of Vata along with Kapha over the skin causes the development of hard nail structures called Charmakeela (nail- like projection from skin surface). These warts are mostly in children and young adults. In the present article, we report a case of 42 years old patient suffering from HPV warts on his forehead and anterior part of the scalp for last 2 years, which increased in size and numbers. Ayurvedic Rasa Aushadhis were prescribed for a period of 21 days in combination, which effectively healed the wart without scar.

Case Report on Ayurvedic Management of Post-Hysterectomy Vaginal Deep Endometriosis

Endometriosis is a disease of contrast, a benign but locally invasive condition characterized by functioning endometrium in sites other than uterine mucosa. It manifests clinically with dysmenorrhea, abnormal uterine bleeding, infertility, dyspareunia, chronic pelvic pain and abdominal pain. Hysterectomy with or without salpingo-oophorectomy is considered the definitive management option in women who completed the family. Persistent or recurrent pain and vaginal spotting after a hysterectomy are one of the most frustrating clinical scenarios in benign gynecology. A 36-year-old parous woman had persistent severe lower abdominal pain, low back ache, and intermittent spotting per vagina after a laparoscopic hysterectomy and left salpingo-oophorectomy for abnormal uterine bleeding with adenomyosis and fibroid. Magnetic Resonance Imaging was suggestive of vaginal deep endometriosis. As the patient presented with yonithoda (lower abdominal pain), krishna alpa thanu ruksha arthava sravam (brownish discharge per vagina), vankshana parswa vyadha (low back ache) and gulma (Hemorrhagic cyst on ultrasonography) it is considered as vathiki yonivyapath. Even though the condition is Vata pradhana, the pain is mainly due to pitha raktha kapha pradhana sopha. So vatanulomana pitha raktha kapha samaka, sophahara and grandhihara chikitsa were the line of management. The symptoms were relieved considerably after one month of treatment and completely cured in the follow-up at the Department of Prasootitantra and Striroga, Govt. Ayurveda college, Trippunithura, Ernakulam, Kerala.

A Clinical Dilemma in the Diagnosis of Dermatofibroma - A Case Report

Dermatofibroma is a button like nodule commonly found at lower extremities. It is common benign tumor of mesenchymal origin composed with fibroblast and histiocytes. In this case a 22 years old male patient came to Shalya Tantra OPD with a complaint of a painless cystic swelling at the right subscapular region. On the basis of clinical findings initially case was diagnosed as a sebaceous cyst. So it was excised under local anesthesia (Lignocaine hydrochloride 2%). The content of cyst was creamish color tissue of firm consistency which was different than sebum. So the excised tissue was sent for histopathology and the wound was sutured. The histopathology report showed Spindle cell neoplasm: benign fibrous histiocytoma. It was diagnosed as benign fibrous histiocytoma.

Ayurvedic Management of Dadru Kushtha (Tinea Corporis)- A Single Case Study

Skin diseases are the major reason behind social stigma and it can affect the physical, psychological and social wellbeing of the patient. Dadru is one of the Kshudra Kustha as per the Charak Samhita and it’s been considered under the heading of Mahakustha as per the reference of Sushruta Samhita. Dadru shows the features of Raga, Pidaka, and Mandala. It is having predominance of Kapha- Pitta Dosha. Though it is highly contagious skin disease as per the reference of Sushruta Samhita it can be compared with fungal infections under the heading of Tinea corporis which is extremely common dermatophyte infection. In all the Samhita Sodhana has been indicated. A 57year old Female patient came with the complaints of Raktabha Mandalotpatti on Nitambha,Hasta Evum Pada Pradeshe (reddish patches on legs,hands and Gluteal region along with Kandu (itching) since last 1year, she was treated with Virechan Karma of Panchakarma along with internal medications. Within two weeks all the lesion were subsided and no any itching and burning sensation were observer over the lesion. Detail description and discussion are discussed in the article.

Management of Mutrashmari (Urolithiasis) with Ayurvedic Formulations - A Case Report

Mutrashmari co-related with Urolithiasis is considered as one of the Ashta-Mahagadas (Eight Dreadful diseases) due to its incurable nature and is contemplated as Yama because it gives intolerable pain. Stone of small size can be easily treated conservatively with Ayurvedic formulations. In this study, a 21-year-old male patient presented with bilateral flank pain, nausea, and dysuria persisting for two months. Ultrasonography revealed a 7 mm calculus in the left kidney at the mid pole, a 3 mm calculus in the right mid ureter, and concretions in all kidney calyces, as confirmed by NCCT(KUB) imaging. The patient experienced substantial symptom relief through the administration of Ayurvedic formulations, namely Pashanbhedadi kwatha churna 5g twice daily, Chandraprabha Vati 500 mg twice daily, and Gokshuradi Guggulu vati 500 mg twice daily throughout the treatment period of 3months. Compliance with dietary recommendations also contributed to the patient's improvement. Subsequent ultrasonography revealed normal findings. Following three months of treatment, the renal calculus was successfully removed, leading to the alleviation of symptoms.

Effect of Unani Formulations in the Management of Unexplained Primary Infertility- A Case Report

Infertility (‘Uqr) is customarily defined as the inability to conceive after 1 year of regular unprotected intercourse. 30% of infertile couples worldwide are diagnosed with unexplained or idiopathic infertility and the problem is defined as the lack of an obvious cause for a couple's infertility and the females’ inability to get pregnant after at least 12 cycles of unprotected intercourse or after six cycles in women above 35 years of age for whom all the standard evaluations are normal. In unani literature various medications have been mentioned for infertility. Here we present a case of primary infertility. The couple were trying to conceive for a period of 3 years. Thorough physical and laboratory investigations were done to rule out the cause which came out as unexplained primary infertility. Unani medications like Safuf Moaene hamal, ma‘jūn-i-mocharas, Khamira gaozaban saada and Sharbat zanjabeel were given continuously for 3 months. Patient got conceived after receiving treatment for 3 months continously. Above mentioned medications have assisted in conception which contains Muqawwi-i- bah activities, chemical constituents like tannins, Lupeol, Shamimicin, withaferins, sitoindosides and various useful alkaloids and multivitamins.

Case presentation of isolated Mayer -Rokitansky -Kuster -Hauser (MRKH) Syndrome- A Short Review of Two Cases

Mayer-Rokitansky-Kuster-Hauser syndrome (MRKH) is a congenital condition with an unclear cause that is characterized by mullerian agenesis and may be linked to cardiac, skeletal, or renal malformations. Most incidents are infrequent. Two typical cases of MRKH syndrome are presented here. When a patient presents with primary amenorrhea, a comprehensive diagnostic workup should encompass hormonal screening, physical examination, chromosomal analysis,.and history collection. A healthy ovary is indicated by normal levels of hormones , follicle stimulating hormone (FSH) and luteinizing hormone (LH). The precise anatomical deficiencies, however, are highlighted by imaging modalities like MRI and USG. In conclusion, the absence of a regular menstrual cycle in a young, healthy-looking adolescent girl should raise the possibility of Mayer-Rokitansky-Kuster-Hauser syndrome. Regarding her potential for conception in the future and the various treatment options (both surgical and non-surgical), the patient should get appropriate counseling.

Siddha Management of Siravatham (Cerebral Palsy)-A Case Study

Siravatham, a disorder affecting the Moolai (the brain) is characterized by the inability to use both upper and lower limbs, walking difficulties, drooling of saliva, speech impediments, heightened fear and anxiety due to vazhi disarrangement. In the Siddha system of medicine, Cerebral Palsy is closely correlated with Siravatham, presenting as a neuromotor disorder in children, with an incidence of 7/1000 in India. This condition results in a permanent dependency of the child on their parents for daily activities, necessitating a holistic therapeutic approach. Excel Medical College and Research Centre is committed to providing medical service to children with this condition, attracting numerous referrals from various parts of India. Siddha, with its specialized branch in child care known as Kuzhanthai Maruthuvam, plays a crucial role in addressing the unique needs of these children. A single case study conducted in the outpatient department from October 2023 to December 2023 aimed to manage Siravatham in a nine-year-old male child. Despite previous treatments by various doctors showing no significant improvement, the parents turned to Siddha treatment for management. This article documents the outcomes of the Siddha treatment, focusing on the child's improvement in angular salivation, vocalization, postural stability and socio-behavioural changes. The findings contribute valuable insights into the effectiveness of Siddha therapy in managing Siravatham.

Management of Cellulitis with Homoeopathic Medicine-A Case Report

Cellulitis is an infection of the deep dermis and subcutaneous tissue, presenting with expanding erythema, warmth, tenderness, and swelling. Streptococci are the most commonly implicated pathogen and often reside in the interdigital toec spaces. Any disruption of the skin surface can allow the organism to invade.The entrance of these microorganisms beneath the skin's surface can cause cellulitis, a severe superficial infection that affects the deep dermis and subcutaneous tissue.The case of cellulitis of 40 years old female has the classic presentation of rubor (redness), dolor (pain), tumor (swelling), calor (heat) the hallmarks of cellulitis.Homoeopathy reduces the need for surgical intervention. This case shows the usefulness of homoeopathic medicine in acute cases.A case of Cellulitis is treated according to the HOM-CASE guidelines with intervention of individualised Homoeopathic medicine CalcariaSulphuricum in ultra dilution. The Cellulitis treated completely without any adverse events.The outcome was assessed along with the photographs.

Plaque Psoriasis Treated Successfully with Individualized Homoeopathic Medicine: An Evidence-Based Case Report

Plaque psoriasis is a clinical condition which although not life threatening, is very difficult to treat. It is the most common clinical type of psoriasis and appears as an immune-mediated inflammatory disease condition. The typical presentation of plaque psoriasis is oval or circular plaques over the scalp, trunk, and extensor surface of the body that become thickened after scratching. Besides the bleeding, painful crack, and scaling due to hyperproliferation of the epidermal cells, approximately 15% of patients with plaque psoriasis develop psoriatic arthritis or other complications. Day by day it is hampering the quality of life of the patients. Misuse of steroidal therapies is increasing the burden of the disease, still, there is almost no evidence of reduced impact of the disease. Factors such as environmental changes, stress, and emotional health disturbances are triggering several co-morbidities. This case report shows a clinically diagnosed case of Plaque Psoriasis which was treated in the Outpatient Department with a single individualized homoeopathic medicine. The mentioned case was treated and recovered by individualized Homoeopathic treatment without recurrence and further complications. Clinically, the improvement of the quality of life of the patient was assessed by the Psoriasis Disability Index. This evidence directs towards a positive outcome of using the Homoeopathic treatment as a standalone therapeutic choice in treating Plaque Psoriasis and the idea of individualization steps forward to the new horizon of futuristic medical science.

Fibre glass Pneumoconiosis and its Management with Homoeopathic Approach- A Case Report

Pneumoconiosis is the spectrum of disorder associated with lungs due to exposure to irritating substances in workplace leading to occupational hazards. It is an irreversible disease occurring due to long term exposure to mineral dust and leads to fibrotic changes in lungs. A male patient of 45 years came to OPD with complaints of breathing distress with stitching type of chest pain which was aggravated on deep inspiration. He would easily get tired from least exertion. The chest pain started for 1.5 years. Along with it he also had dry tickling cough and flatulence with tenderness in right hypochondrium occasionally. He had been working in fibre-glass industry for more than 16 years. The patient complained of breathing difficulty with chest pain of stitching character which aggravated on deep inspiration. The patient had a history of working in fibre-glass industry for more than 16 years and was diagnosed as fibre glass pneumoconiosis pulmonary with ground glass opacity in chest X-ray. In this case report, Bryonia alba in a series of LM potencies was prescribed considering the repertorial totality and analysis of causative modalities. The patient was having alleviation in the respiratory symptoms and pathologically Chest X-ray revealed no abnormality after a span of 12 months. Many chronic cases with irreversible pathological changes have been successfully treated with individualized homoeopathic medicines. Though such improvements are rarely documented in homoeopathic literature, a well-designed clinical trial to assess the effectiveness of homoeopathic medicines in spectrum of pneumoconiosis could establish its worth in such incurable diseases. This documentation will be a decent addition in medical literature showing the efficacy of homoeopathy in such cases.

Exploring Sulphur’s Efficacy in Treating Lichen Planus: A Case Report

Lichen planus is an inflammatory dermatosis characterized by violaceous, scaly, polygonal flat-topped papules involving the flexor aspects of the wrists, lower limbs and the genital and oral mucosa’s. Lesions generally clear up within a few months to years, leaving areas of hyperpigmentation. LP affects patients of all ages, but up to 95% of all cases occur in adults, with most patients presenting between the third and sixth decades of life. Clinical experience and studies identify a positive role of Homoeopathy in skin conditions Lichen planus. This case treated with individualized Homoeopathic medicine over a period of 6 month, shows significant improvement which is evident from the photographs. This case shows the usefulness of Homoeopathy in treating the Lichen planus.

Homoeopathy in the Management of Suspected Case of Papillary Thyroid Carcinoma - A Case Report

Papillary thyroid carcinoma is an epithelial malignancy showing follicular cell differentiation and distinctive nuclear features. It is the predominant form of thyroid cancer, accounting for 80 to 85% of all thyroid cancer cases. The tumor appears as an irregular solid mass, about 10% as metastatic disease at initial presentation. A lobectomy is an option for unifocal tumors smaller than 4cm with no evidence of extrathyroidal extension. A 21-year-old female, consulted in Endocrinology OPD of NHRIMH, Kottayam on 4 February 2020 with weakness of body, hairfall, hypopigmented patches on skin, headache on forehead while travelling and eye straining, ameliorated by vomiting. She had Thyroiditis with a solitary hypoechoic nodule mm) in left lobe of thyroid gland, TBSRTC Category V and Suspicious for Papillary carcinoma as per the investigation on 25 July 2019. Elevated Serum ATG (575 IU/ml), ATPO (44.5 IU/ml). After thorough case-taking, homoeopathic treatment with Phosphorus 30, Calc Phos 200, 1M and Thyroidinum 1M led to remarkable improvement assessed using Modified Naranjo Criteria. With 27 months of individualized Homoeopathic treatment, No definite focal lesions noted and both lobes showed only features of Thyroiditis as per the investigation on 22 May 2023.

Treatment of Eczema by Homoeopathic Medicine- A Case Report

Eczema is a common skin condition that can itch, peel, ooze or form vesicles in both adults and children. Modern medicine uses corticosteroids and emollients for its treatment, which sometimes may cause unwanted side effects. In contrast, Homoeopathy plays a beneficial role in the long-term treatment of eczema. In this case report, Homeopathic medicine Hepar sulph was given to a male patient with eczema on hands and legs on the basis of individualization that leads to significant improvement of the complaints. This case report demonstrates the effectiveness of homoeopathic medicines in treatment of eczema.

Individualized Homoeopathic Treatment of a Geriatric Case of Goitre- A Case-Report

Goiter is defined as an enlarged thyroid gland (>20-25g), that can be diffuse/simple or nodular. Thyroid nodules are common, perceived by palpation in at least 4 percent of the population in iodine-sufficient areas and more than half of patients by using ultrasonography, with the majority being asymptomatic. Nodular goitre is one of the most common endocrine problems in the world today, affecting up to 12% of the world's population and 170 million people in India. Females are more likely to suffer from goitre than males of the same age group with a sex ratio Female: Male = 24:1. Diagnosis of a case or goitre is confirmed by ultrasonography and/or thyroid scintigraphy. The conventional approach to treat goitre is to wait and watch when it is asymptomatic along with an annual thyroid function test as the natural history is progression to a toxic multinodular goitre. This case report summarizes the improvement seen in a 72-year-old lady diagnosed with cystic colloidal goitre with calcification and thickened isthmus (volume of the right lobe was 25cc with calcification of 0.7cm & 0.5 cm, volume of the left lobe was1.8cc) with the help of Causticum 200, which was chosen guided by the principles of homoeopathy, and was prescribed only once during treatment. At the end of 9 months after the commencement of treatment, radiological reports showed the volume of the right lobe of the thyroid was 2.94cc, the volume of the left lobe was 1.32 cc, the isthmus was normal, both lobes showed evidence of calcification but exact measurement was not available. Clinically goitre was not visible to the naked eye. Thus, the effectiveness of the minimum dose of the correctly chosen simillimum is evident, as observed in this case.

Role of Individualised Homoeopathic Medicine in Diabetic Foot Ulcer Management: An Evidence-Based Case Report

Diabetes Mellitus (DM) is one of the major health issues nowadays. Patients with diabetes are prone to many complications and diabetic foot ulcer (DFU) is one of them. It is a very common complication of poorly managed diabetes. As a result, skin tissues break down and expose the layers underneath. Although there are many standard managements available, the healing rate of diabetic foot ulcers remains low. Amputation is the ultimate choice in modern medicine but homoeopathy can be beneficial in some cases. Homoeopathy provides a good scope in the treatment of diabetic foot ulcers and prevents amputation. This is a case of a 67-year-old male patient suffering from an ulcer in the right great toe for almost 7 months. There was irritation and burning present in the affected area with a whiteish offensive sticky discharge of pus. Also, some blackish tissue surrounding the ulcer. Symptoms were ameliorated by hot applications. The patient was prescribed the indicated individualised homoeopathic medicine Arsenicum Album based on the totality of symptoms. Four doses of 200^th in centesimal potency was prescribed which gave a positive result by the end of one and half month.

Role of Individualized Homeopathic Intervention in Management of Herpes Simplex: An Evidence-Based Case Report

Infection with Herpes Simplex virus is very extensive in the human population. Virus replication is generally limited to the epithelium but reactivation of the latent virus in the enervating sensory neurons not only produces recurrent localized lesions, but also encephalitis, keratitis, blindness and many other severe systemic diseases. Overall prevalence varies between 40-80% and 0-40% for HSV 1 and HSV 2 respectively. The primary and recurrent cases are treated by anti-viral therapies in modern medicine. But in today’s world, resistance to the available drugs, toxicity due to long term drug use causes much difficulty to treat the cases. Several HSV vaccines are also being terminated due to much controversy. This case report of a patient, complaining of pain redness with itching & blister formation over chest and back for last 5 months, shows a case of Herpes Simplex virus infection, which was treated and recovered with individualized Homeopathic rithout recurrence and further complication. Clinical improvement as well as the overall wellbeing directs towards the success of applying single Homeopathic medicines to the individual. The evidence of recovery in the case of HSV infection as well as other difficult to treat cases indicates to the new horizon of the future medical guidelines.

In some of cases congenital anomalies can be detected during this those are symptomless. In our teaching and research Hospital ITRA Jamnagar A case of bilateral extra cervical rib as diagnosed during x-ray chest. The case was treated for haemorrhoid surgery successfully without any issue with the extra rib as patient was symptom less.

A 50-year-old male without comorbidities came to the Shalya Tantra OPD (Ayurveda Surgery) on foot with the primary concern regarding swelling over forehead gradually growing in size since 17 months. He had underwent deceleration injury before 18 months while traveling in the bus. Initially swelling was small but gradually enlarged without any pain and other symptoms. Ultrasonography findings suggest sub-galeal lipoma, But clinical summary supports the sign and symptoms of pseudolipoma. The swelling was excised and sutured as per conventional surgery. Ultrasonography findings showing sub-galeal lipoma has characteristics like firm, fixed, immobile mass. ^[1] But in the clinical findings the swelling was semisolid, mobile, movable and positive slip-sign. The traumatic history over forehead supports the diagnosis of post traumatic pseudo-lipoma.
